Kids at the Ridge Program Registration Form
Registration is required for all Children's Programming for kids attending Nursery to Grade 6. This must be completed annually for insurance purposes. We are collecting this information to register your children for all of our Children's Ministry Programs (Little Lambs Nursery, SONSeekers and The Wave). Your information will also be used to inform you of upcoming Children's Ministries Activities & Events.
Please NOTE! This form has the ability to register families with multiple children! Once you have entered your child's information in the CHILD INFO section, you can click "Return to Form" to enter additional children.
You MUST click SUBMIT on each Child's Registration Info before clicking the Submit button at the very bottom of the form! (if you do not do this, you will lose information)

PERMISSION FORM AND CONSENT
Whyte Ridge Baptist Church is collecting and retaining this personal information for the purpose of enrolling your child in our programs, to assign the student to the appropriate classes, to develop and nurture ongoing relationships with you and your child, and to inform you of program updates and upcoming opportunities at our church. This information will be maintained permanently as it is a requirement of our insurance company and legal counsel. If you wish Whyte Ridge Baptist Church to limit the information collected, or to view your child’s information, please contact us.
Parent / Guardian Options
I have read, understood, and agree with the above and sign it to cover all children’s/youth program activities for the program year effective as stated below. A separate Informed Letter of Consent will be sent home for off-site activities and activities of elevated risk.
